She has a successful track record in the planning and execution of activities that improve operational and clinical outcomes and her expertise has been acknowledged as an invited presenter at national level symposia.

Rehan Shah, MD

Chief Medical Officer/President
Rehan Shah, MD received his training at SUNY Downstate Medical Center. He has gained valuable experience by treating a very diverse population with vast pathology in the New York area. He has served as Medical Director of Dialysis units giving him first hand knowledge and expertise in Kidney Disease and Dialysis.

He currently is Founder and CEO of Renviva, a Physician owned and Led Dialysis Provider focused on bringing Dialysis services to the Home.

Dr. Rehan Shah is passionate about educating patients and firmly believes that education improves patient outcomes. Therefore, he teamed up with Industry leaders to form Creative Health Media ( CHM). CHM leverages technology to allow education to be scaled throughout the Kidney Disease community.

Gary Holderby

Gary has over 20 years of experience in founding and building high growth, emerging technology companies. He has held multiple CEO and COO level roles and thru his extensive experience in the healthcare technology market, has worked with leading companies across many major verticals including payer, provider, pharmaceutical, pharmacy, population health, EHR/EMR, technology providers, and not-for-profit foundations.

Most recently, Gary worked with the American Heart Association to establish and launch their Center for Health Technology & Innovation. Thru his extensive network, Gary continues to invest in innovative healthcare technology companies and is an active board/advisory member in the industry.

Patrick Dunn, PhD, MS, MBA

Is a Senior Program Manager for Connected Heart Health and the AHA Inside initiative for the American Heart Association’s new Center of Excellence for Health Technology and Innovation. Dr. Dunn is a clinical exercise physiologist and health educator with 33 years of experience in cardiac rehabilitation, preventive cardiology and wellness. His research interests are in the development of health literacy and self-management skills for patients with cardiovascular disease and diabetes, and is the founder of Your Heart Score, LLC.

John trains transplantation professionals, patients and donors to increase living donors. Thereby, saving lives of patients and increasing revenues for transplant centers. I am a living donor kidney recipient. Because of the struggles I endured on my journey for a transplant, I researched the lack of donors and found a gap in educating potential donors. I have developed a non-clinical educational program for potential donors, patients and transplantation professionals called “Proactive Education for Family and Friends”.

My passion to save lives of kidney patients has motivated me to establish Living Donor Outreach, a 501(c)3 non-profit California corporation. I have over 3 decades of experience working with non-profits in the U. S. and internationally.

Dr. A. Oussama Rifai

Dr Rifai is a practicing kidney specialist for more than 20 years with main focus of early recognition and prevention of kidney disease, delaying the progression of Kidney disease and education for patient with advanced kidney disease. He is board certified in internal Medicine and Nephrology as well as a recognized hypertension specialist by the American Society of Hypertension.

His passion for reach out is evident by establishing an online guide for patients and addressing issues patients ask about, since he has collected all the questions and concerns over a period of two decades.

He is the Vice President of Medical Affairs, for "The Virtual Nephrologist".

27 Year Chronic Kidney Disease Warrior!

Dawn has experienced first-hand every renal replacement modality. Including a kidney transplant and rejection. As a result, she has a plethora of insight into the needs of people with Chronic Kidney Disease and the professionals who take care of them. She is dedicated to improving the quality of life of consumers of renal replacement therapy with Education, Encouragement and Empowerment .

She is a communicator, mentor and educator with over 20 years of experience in advocacy and patient/provider education. She serves on many boards and committees locally and nationally and is the CEO of her own patient-run organization, the New York State CKD Champions.

Scott Burton

Scott Burton is a creative and passionate visionary, working for the last 10 years in marketing and business development consulting. A lifelong kidney patient, he was born with a posterior urethral valve blockage that damaged his kidneys, with doctors not expecting him to live 2 days, Scott spent his childhood in and out of the hospital waiting for the day when dialysis would be required. He first went on peritoneal dialysis at the age of 12, waiting 4 years for his first kidney transplant. Countless episodes of rejection and finally losing his transplant 4 years later in January 2003. Scott has spent the last 17 years on hemodialysis and is on the list for a second transplant, but due to being highly sensitized, he is a very difficult match. Scott has juggled day to day life around dialysis, going back to school full time in 2006 and getting a bachelor’s degree in media studies – film/video production from UNCG in 2010, self-publishing a book of poetry – telling his own medical struggles through poetry and prose, working with clients in marketing and business development consulting and is currently pursuing his MBA at UNCG.

Having lived this from day one, in 2016 he set out to turn what many would consider a negative into a positive by launching The Forever is Tomorrow Foundation, a nonprofit organization dedicated to raising awareness, educating and building a resource for both the public and patients and their loved ones through multiple web video series & local events.”
